Mobile Form Builder. Smart forms, right in the mobile app.
Build forms that load in the mobile app — forms that connect to vitals, forms that trigger from the Portal Scheduler, forms that pop up based on a patient’s evolution. Cerbo’s questionnaire system is fantastic, so we leveraged it to do exactly that.
Build it → loads in the app → linked to vitals, scheduler & evolution
—— Why We Built It
A closed door, and a brilliant one right next to it.
Cerbo’s patient forms aren’t accessible via API — so the obvious path was closed. But Cerbo’s questionnaire system is fantastic, and that was the door we walked through instead. This is Leverage in its purest form: using Cerbo’s own power and architecture to do something it was never designed to do on its own.
The result is a true mobile form builder. Build forms that load in the mobile app — forms that connect to vitals, forms that trigger from the Portal Scheduler, forms that pop up based on a patient’s evolution. Not static PDFs, but living forms that respond to where the patient actually is.
—— What It Does
Forms that know when to show up.
Loads in the mobile app
Forms render natively in the patient’s mobile experience — easy to complete on the device they already have in hand.
Connects to vitals
Fields can link to vital signs — so a form isn’t just collected text, it’s tied to the real clinical data behind it.
Triggers from Portal Scheduler
Booking an appointment can fire the right form automatically — the questionnaire arrives because the visit was scheduled, no manual sending.
Pops up on patient evolution
As a patient’s situation changes, the relevant form surfaces at the right moment — the questionnaire follows the clinical story.
Leverages Cerbo’s questionnaires
Built on top of Cerbo’s excellent questionnaire system — we didn’t replace it, we extended it to reach the mobile app and beyond.
Grounded in the Cerbo record
What patients submit flows back to where it belongs — the data stays connected to Cerbo, not stranded in a separate form tool.
This Is Leverage
Doing what Cerbo wasn’t designed to do — with Cerbo.
The API door was closed, but the questionnaire system was brilliant. Rather than work around Cerbo, we worked through it — turning a fantastic feature into mobile forms that connect to vitals, react to scheduling, and follow a patient’s evolution. That’s leverage: amplifying Cerbo’s strengths into something entirely new.
Build the form. Let it find the moment.
Cerbo is the best EHR in functional medicine. Always has been. Always will be.
