Appointment Form Notification. The right forms, done before they arrive.
One of the most powerful applications we’ve ever built. Each appointment knows which forms it needs, reminds the patient on a steady cadence — 14 days out, then 10, 5, and 2 — and if anything’s still missing, it hands a task to your admin. A held container of support from scheduling all the way to the visit.
14 → 10 → 5 → 2 days → then an admin task if needed
—— Why We Built It
The form should be done before the patient sits down.
Different appointments need different forms. Some are single-entry, like a family history. Some are multi-entry, like an MSQ. Forms also change over time, so the system includes version detection to make sure the patient completes the current one — not last year’s.
So we built a steady, caring cadence: a reminder 14 days out, then 10, 5, and 2 — and if the forms still aren’t done, it automatically creates a task for your admin to follow up personally. From the moment of scheduling to the moment of the appointment, the container of support is held the whole way.
—— What It Does
Smart about forms. Relentless about follow-through.
The right forms per appointment
Each appointment type knows exactly which forms it requires — so patients are never asked for the wrong ones, or left missing the right ones.
Single- and multi-entry
Handles a one-time form like a family history and a repeating one like an MSQ — each behaving the way that kind of form should.
Version detection
If a form has been updated, the system makes sure the patient fills out the current version — no stale data slipping into the chart.
A 14 / 10 / 5 / 2 cadence
Reminders arrive on a thoughtful rhythm — early enough to be easy, persistent enough to actually land before the visit.
Auto-task for admin
If the forms still aren’t done as the date nears, a task is created for your team — so a real person closes the gap, every time.
A held container of support
From scheduling to the appointment, the patient is gently carried toward readiness — never left to remember on their own, never dropped.
From Scheduling To The Visit
The whole way there, held.
This is what “ready” should feel like: the right forms, the current versions, a humane cadence, and a human backstop. The patient arrives prepared and the provider opens a complete chart — because nothing was left to chance in between.
Forms ready. Visit ready.
