JotForm. Same idea as TypeForm — on the platform you run.
Same philosophy as our TypeForm integration, different form platform. A practice runs JotForm, and on patient conversion every form injects automatically into the Cerbo chart — then new submissions flow in continuously. Three platforms (TypeForm, JotForm, Gravity) solving exactly the same problem.
Becomes a patient → forms inject · new ones keep flowing
—— Why We Built It
The problem doesn’t care which form builder you chose.
It’s the same story as TypeForm, on a different platform. A practice uses JotForm to collect prospect data, and they shouldn’t have to switch tools to get that data where it belongs. So forms inject automatically into the Cerbo chart on patient conversion, and new submissions flow in continuously after that.
That’s the real point: three different platforms — TypeForm, JotForm, and Gravity — solving the same problem. Whichever form builder a practice already lives in, the outcome is identical: the answers end up in the chart, automatically, forever.
—— What It Does
Your form builder. Our same dependable outcome.
Keep collecting in JotForm
Run intake, history, and symptom forms in JotForm exactly as you do today — no migration, no relearning.
Inject on conversion
When a prospect becomes a patient, every JotForm they submitted injects into their Cerbo chart automatically — the whole history, on file.
New submissions flow continuously
It doesn’t stop at conversion — any later JotForm a patient completes flows into the chart on its own, keeping the record current.
Three platforms, one solution
TypeForm, JotForm, and Gravity all reach the same destination — pick the builder you prefer; the integration behaves identically.
No re-asking, no re-typing
Patients never repeat what they already answered, and staff never re-enter it — the submissions move themselves.
Lands in Cerbo
Whatever the builder, the form data ends up attached to the patient in Cerbo — where care actually happens.
One Problem, Solved Three Ways
We meet your forms wherever they live.
TypeForm, JotForm, Gravity — the platform is a detail; getting the answers into the chart is the point. Rather than insist on one builder, we built the same clean outcome for each, so the tool your practice already chose is never the reason data gets stuck outside the record.
Any form builder. Always in the chart.
Cerbo is the best EHR in functional medicine. Always has been. Always will be.
