Gravity Forms. Right inside the WordPress you already run.
For practices living on WordPress with Gravity Forms. Same logic as our TypeForm and JotForm integrations: form submissions sync to Cerbo on patient conversion, and new submissions flow continuously after that. We build per segment, because different practices live in different ecosystems.
WordPress forms sync on conversion · new ones keep flowing
—— Why We Built It
Meet practices where they already live.
Plenty of practices run their whole web presence on WordPress, with Gravity Forms handling intake. Asking them to leave that ecosystem just to connect their forms would be the wrong trade. So we built the same integration logic for Gravity that we built for TypeForm and JotForm.
The behavior is identical: form submissions sync to Cerbo on patient conversion, and new submissions flow continuously afterward. We build per segment because different practices live in different ecosystems — and the one you’re already in should never be the obstacle.
—— What It Does
WordPress in, chart out.
Native to your WordPress site
Keep collecting with Gravity Forms inside the WordPress you already manage — no new platform, no second place to maintain.
Sync on conversion
When a prospect becomes a patient, their Gravity Forms submissions sync into the Cerbo chart automatically — the full history, on file.
New submissions flow continuously
Every later Gravity form a patient completes flows into the chart on its own — the record stays current without anyone tending it.
Built per ecosystem
TypeForm, JotForm, Gravity — we built each for the segment that lives in it, so the right practices get a native fit, not a workaround.
No re-asking, no re-typing
Patients never repeat what they answered on your site, and staff never re-enter it — the submissions simply move themselves.
Lands in Cerbo
Wherever the form was built, the data ends up attached to the patient in Cerbo — the system of record where care happens.
Different Ecosystems, Same Promise
You shouldn’t leave your stack to connect it.
Some practices live in WordPress; others don’t. Rather than force everyone onto one path, we build per segment — Gravity for the WordPress world, TypeForm and JotForm for the rest — so the promise is the same no matter where you work: your forms end up in the chart, automatically.
Your ecosystem. Cerbo’s chart.
Cerbo is the best EHR in functional medicine. Always has been. Always will be.
